FNP 8447 - Primary Care for the Chronically Ill 2
4 Credits
Focus of this course is on the role of the DNP-FNP as a member and leader of interprofessional teams in chronic illness care. The interaction between chronic illness and the health care system is analyzed. Models of care such as home health care, long-term care, palliative care, and rehabilitation are explored. THE DNP-FNP’s role as a care provider and policy leader in these systems is examined. This courses requires 90 clock hours in clinical practicum.
Prerequisite(s): Student must have taken FNP 8443 .
